In January 2023, a 34-year-old man named Hugo Flint-Cahan was killed by another patient at Newham Mental Health Centre in east London. A coroner's investigation concluded that staff neglect had contributed to his death. The inquiry revealed that on the night of the incident, staff members were sleeping and using their phones rather than monitoring patients. A coroner's report has now warned the mental health trust that further deaths may occur unless serious staffing and procedural issues are resolved.
